TENS Machine Rentals

TERMS OF RENTAL AGREEMENT

This agreement for the Elle TENS 2 or the TensCare PerfectTENS machine is made between Laura Bradley and the lessee.

1. This equipment remains the property of Laura Bradley. Lessee has no rights to such equipment except expressed in this agreement.
2. Lessee agrees to pay the rental fees in accordance with the fee structure established shown under the rental payment plan. The rental fee is due in advance.
3. The rental rate for the equipment does not include applicable tax
4. Lessee agrees not to move the equipment out of this province without consent of Laura Bradley
5. Lessee agrees to allow no person other than the lessee to use the equipment provided.
6. Lessee agrees to return the equipment in clean condition. If the equipment is not clean, the lessee agrees to pay Laura Bradley a minimum cleaning fee of $30.00.
7. Lessee agrees to return the equipment in good repair. If the equipment is not in good repair, the lessee agrees to pay the rental station a minimum charge of $50.00.
8. Lessee agrees to pay all fees, including collection and court costs, if this agreement is not fulfilled and/or the renter fails to return the equipment as set forth herein and/or pay contract in full.
9. Lessee acknowledges that Laura Bradley is permitted to charge the credit card of the lessee the value of lost, damages or non-returned equipment.
10. Upon signature of lessee, it is acknowledged that the equipment has been delivered in good condition.
11. Lessee acknowledges that this is a rental and not a purchase and no refunds will be issued for an order placed in error. 
12. Laura Bradley will offer free drop off and collection in Toronto. All other locations with be charged at $0.50/km

Birth Pool Rentals

TERMS OF RENTAL AGREEMENT

This agreement is for the rental of a Birthing Pool (including all accessories such as liner, hoses, air pump, and water pump) is made between Laura Bradley and the lessee.

1. This equipment remains the property of Laura Bradley. Lessee has no rights to such equipment except those expressed in this agreement.
2. Lessee agrees to pay the rental fees in accordance with the posted fee structure. Fees are due in advance of equipment delivery.
3. Applicable taxes will be added to the rental rate.
4. Lessee agrees not to move the pool or supplied equipment outside of the province without consent of Laura Bradley.
5. Pool and accessories are for use exclusively by the named lessee and household, and cannot be transferred or shared outside the home.
6. The pool must always be used with the provided disposable liner. Never reuse a liner.
7. Upon return, lessee agrees to return all reusable equipment in clean, dry condition. All disposable items (liner, debris net, etc.) must be properly bagged for disposal and are not to be returned.
8. If equipment is not clean, lessee agrees to pay a cleaning fee (cost depends on state of pool upon return). If equipment is damaged, lessee agrees to pay fee for the replacement value of affected equipment. (up to the cost of a new pool if the pool needs replacing)
9. Lessee agrees to pay any fees, including collection and court costs, if terms are not fulfilled and/or equipment is not returned as required.
10. Lessee acknowledges that Laura Bradley is permitted to charge the credit card of the lessee for lost, damaged, or non-returned equipment.
11. Lessee certifies equipment has been delivered in good condition and will report any problem immediately upon receipt.
12. No refunds will be issued for an order placed in error.
13. Laura Bradley provides free drop-off and collection in Toronto; other locations are charged at $0.50/km.
14. 15. Lessee is responsible for reading and following all provided instructions and waives liability for any injury, damage, or health complication resulting from misuse, improper setup, or failure to follow instructions.
15. Lessee will not hold Laura Bradley liable for any undesirable or unpredictable outcome related to the use of the birthing pool and accessories. Laura Bradley’s liability is limited to repair or replacement of equipment found defective under normal use.
16. Health and Safety Protocols: 

The lessee agrees to follow all included safety protocols, including:

• Using the pool exclusively indoors and away from sharp objects, pets, or unsupervised children.
• Maintaining water temperature between 36–37°C and never overfilling the pool.
• Keeping all electrical cords and equipment a minimum of 3 meters (10 feet) away from the filled pool.
• Cleaning all supplied reusable equipment prior to return using approved protocols (bleach/water for pool, disinfectant wipes for pumps and hoses) and drying all items thoroughly.
• Bagging and sealing any disposable items that come in contact with blood or body fluids for safe disposal.
• Wearing gloves when cleaning or handling used pool components.

Breast Pump Rentals

TERMS OF RENTAL AGREEMENT

This agreement for the rental of breast pump equipment (make/model specified in rental form) is made between Laura Bradley and the lessee.

1. This equipment remains the property of Laura Bradley. The lessee has no rights to such equipment except those expressed in this agreement.
2. Lessee agrees to pay the rental fees as per the rental payment plan posted. Rental fee is due in advance.
3. The rental rate for the equipment does not include applicable tax.
4. Lessee agrees not to move the equipment out of the province without written consent from Laura Bradley.
5. The equipment may only be used by the named lessee and within their household. No other individuals may use the equipment.
6. All accessories supplied for pumping (tubing, flanges, bottles, membranes, etc.) that are designated as single-use/disposable are for the lessee’s use only and must not be returned.
7. Lessee agrees to return the pump in clean condition. Cleaning is the responsibility of the lessee: wipe down the exterior of the machine, disinfect all non-disposable components, and ensure it is dry prior to return. If the pump is not clean, the lessee agrees to pay Laura Bradley a minimum cleaning fee of $25.00.
8. Lessee agrees to return the equipment in good repair. Excessive damage, loss, or missing parts (excluding normal mechanical failure or regular wear) will be charged to the lessee at a minimum fee of $50.00 or the replacement value.
9. Lessee agrees to pay all fees, including collection and court costs, should this agreement not be fulfilled and/or renter fails to return the equipment or pay all contract amounts.
10. Lessee authorizes Laura Bradley to charge the credit card on file for lost, damaged or non-returned equipment up to the replacement value stated prior to rental.
11. Upon signature, lessee acknowledges equipment has been delivered in good working order and will report any concerns immediately upon receipt.
12. Lessee agrees that this is a rental and not a purchase—no refunds will be issued for orders placed in error.
13. Laura Bradley offers free drop off and collection in Toronto. Additional locations are charged at $0.50/km.
14. Rental warranties: Laura Bradley agrees to repair or replace any equipment that becomes defective under normal use. Liability is limited to repair or replacement; no incidental or consequential damages are covered.
15. Health and Safety Protocols: 

Lessee agrees to:

• Only use provided single-use or new sterilized accessories.
• Wash hands before each use and after cleaning or handling pump accessories.
• Disinfect any pump components that come in contact with milk or body fluids according to manufacturer instructions.
• Bag and seal any reusable components exposed to blood or body fluids for safe disposal or return, wearing gloves as needed.
• Promptly report any malfunction, exposure to contaminants, or other health/safety incidents.

LAURA BRADLEY WARRANTS TO REPAIR OR REPLACE ANY EQUIPMENT WHICH IS OR BECOMES DEFECTIVE UNDER NORMAL USE. LAURA BRADLEY WILL NOT BE LIABLE FOR INCIDENTAL, CONSEQUENTIAL OR ANY OTHER DAMAGES RESULTING FROM DEFECTIVE EQUIPMENT; THEIR LIABILITY IS LIMITED TO REPAIR OR REPLACEMENT OF THE EQUIPMENT
